I'm using mizudroid 3.7.4 on a samsung galxy S8. is it possible to get mizidroid to run in the background? - without having it live on the screen all the time?

Yes, it is possible with push notifications.
Push notifications should work by default if your VoIP server has support for it (direct) or if your VoIP server is public (via our SIP-push gateway).
Push notifications can be enabled/disabled from the MizuDroid app settings (auto by default).
