Also with the latest 3.4.29 release the app is unusable because the phone rings only when the phone is unlocked and the app started.
I see this behaviour both when running in standlone mode and integration mode.

some details:

- I open the app: the phone registers to my asterisk server
incoming calls are possible

- I push the stdby button of the phone
after a about 20 seconds asterisk says "Unregistered 'myphone' " and then calls to the phone are not going through anymore.

- phone is Samsung Note 10+ with Android 10

Maybe somebody has an idea how to tell the phone NOT to kill/disable the app in stdby mode?
Btw: during app initial configuration I was requested to enable "foreground mode" ... but I have no idea where I can do that. Maybe this is exactly the root of the problem?

Yes, of course MizuDroid needs to be running in the background or foreground to receive calls, if push notification is not being used. This can be configured in Settings > Advanced Settings > General Settings > Run in background.
